Paper applications available upon request. Once an application is approved by the Market Manager you will be required to submit the following: payment, a W9 Form, Liability Insurance with the City of Newton, 1000 Commonwealth Ave. Newton, MA 02459 listed, as an additional certificate holder and if required, a Seasonal Food Permit from the City of Newton Health Department.
Newton Food/Propane Permits
This application is necessary for any individual or corporation who wants to serve food in the City of Newton for up to 14 days (temporary) or up to six months (seasonal) which includes the Newton Farmer’s Market. All food permit holders must be in compliance with the MA State regulations 105 CMR 590.00.
Please note:
To complete this application, you will need the following:
- Proposed food menu
- Food preparation plans
- Food Manager Certificate
- Allergy Awareness Certificate
- Contact Information for Person-In-Charge during event
- Fire Permit, if using propane
- Wholesalers/Catering license, if applicable
